This form must be completed for all horse movements into Victoria from both New South Wales (NSW) and Queensland (QLD) from both White and Green Zones.

The details entered on this form will be recorded and you will be presented with the Victorian Horse Entry Declaration (VHED) certificate on the screen. You must print one copy of the VHED, then sign and date it. The declaration is not valid until it has been signed and dated.

Journey details
A new Victorian Horse Entry Declaration must be completed if multiple trips are not completed within 30 days, or if the horses originate from a different destination or if different vehicles or routes are used.
If multiple trips are required between the same origin and the same destination over a 30 day period, tick ‘Multiple trips’, put the first departure date in ‘Departure Date' box and the last arrival date (maximum 30 days) in ‘Arrival Date' box. A signed and dated copy of the completed Victorian Horse Entry Declaration must be carried with each trip into Victoria and a new Victorian Horse Entry Declaration must be completed every 30 days.
